To better serve the community and streamline efforts, PRF will require all applicants to come in person to our campus at 1401 E. Jefferson Street, Phoenix, AZ 85034 to complete an application. Walk-in applicants are welcome Monday through Friday from 9:00 AM to 4:30 PM. Proper identification and original paper copies of supporting documents for rent & utilities are required as part of the verification and application processes. Required documents for verification must be available and presented when the application is completed. Applicants may contact the Human Services at 602-257-4575.
Providing your information and requesting assistance is voluntary. All applicants who request assistance do so without any implied or actual guarantee that Pilgrim Rest Foundation will approve the request.

Failure to complete the application may result in rejection of the request. Applications are reviewed weekly. Applications submitted after 2pm Tuesday of each week will be considered for the following week.
